Winter shunt needle
Large-bore (18G) Tru-Cut-type biopsy needle introduced percutaneously through the glans to create one or more fistulous communications between the glans and each corpus cavernosum.
How it is used
First-line surgical shunt for ischemic priapism refractory to corporal aspiration and intracavernosal sympathomimetics.
Clinical tip
Reported success is only ~50-65%, and performing the shunt within 24h of onset is associated with higher rates of resulting erectile dysfunction; early penile prosthesis should be considered in prolonged, severe cases.
